What is a D4, DVLA or Driver Medical?

It is a medical examination to ensure that you meet the DVLA group 2 standard and it consists of the following:

  • An eyesight test using a Snellen Chart
  • Blood pressure checks to ensure your BP meets the required criteria, and
  • A questionnaire regarding your medical history.
Will my eyesight be tested?

Yes, this is part of the medical.

Can I use my glasses or contact lenses to pass the eye test?

Yes, absolutely.

Do I need a medical to obtain a C1 license?

Yes, to operate a small lorry between 3.5 and 7.5 tons, or a small bus like a minibus, you must possess C1 license. This requires a medical.

What do I need to bring to my Driver Medical?

Bring the following items for your medical:

  • D4 Medical Form (available to download here)
  • a photo ID.
  • If you use glasses or contact lenses to drive then bring glasses with you, and
  • If you use any medication then bring a medication list with you.
What do I need to bring to my Taxi Medical?

Bring the following items for your medical:

  • Relevant Taxi Medical Form
  • Medical records or have access to them via the NHS app
  • a photo ID.
  • If you use glasses or contact lenses to drive then bring glasses with you, and
  • If you use any medication then bring a medication list with you.

 

Who can fill in a D4 form?

Any Doctor who is licensed to practise medicine in the UK and holds full registration with the General Medical Council (GMC), can complete the DVLA D4 Driver Medical.

This can be a GP or a private firm like South Wales Driver Medicals whose Doctors specialise in Driver medicals.

The page 2 of the D4 form can be filled out by an optometrist or optician, but most good doctors will provide a free eye exam to avoid the expense of seeing an optician.

No, absolutely not!

How long is the completed D4 medical form valid for?

The completed D4 medical form is valid for four months. If it does not reach the DVLA within four months, they will not process it. 

What is the ealiest, in advance of my HGV/PCV renewal date, I can have my medical?

You can apply for renewal of your license three months in advance.

How often do HGV or PCV drivers need medicals?

You need a medical when you first apply for a HGV or PCV license. Then at age 45 and every 5 years thereafter until you reach age 65. From age 65 you need a medical every year.

How long can I drive with an expired driving license?

The Section 88 is used sometimes to allow some people to continue driving after their license has expired. This applies to drivers who are medically fit to drive and have submitted an application to renew their licence.

Can any doctor do a taxi medical?

This differs across different taxi licensing authorities. Some allow you to go to any doctor, others  require you to go to the approved council doctor, and some authorities say that you must have the medical with your own GP, so you need to check with your licensing officer before you book your appointment.
